Q2 2026 IVD results tell a story of accelerating differentiation. The macro backdrop pressured nearly everyone: continued China Volume-Based Procurement (VBP) pricing pressure, a mild Northern Hemisphere respiratory season, and real structural change moving through Specialty Diagnostics. But the businesses converting clinical credibility, regulatory endorsements, and platform investment into durable competitive moats are pulling meaningfully ahead of those still exposed to a single category or geography.


Three cross-segment themes dominated the quarter.

1 . China's Volume-Based Procurement pricing policy continued to compress performance for multinational players with meaningful China exposure. Siemens Healthineers (-10.3% China), QuidelOrtho (-23% China), and Sysmex (-33.8% China local currency) all reported steep China declines that mask genuinely healthy performance outside the region.


2. Regulatory and clinical endorsements are becoming real competitive moats, not just marketing claims. The American Cancer Society's May 2026 reaffirmation of Cologuard as a preferred colorectal cancer screening option, Roche's first-to-market VENTANA companion diagnostics status, and Waters' FDA clearance of BACTEC FXI all convert clinical credibility directly into commercial defensibility.


3. Specialty Diagnostics saw the most consequential structural movement of any segment. Thermo Fisher's pending microbiology divestiture, Roche's $750M acquisition of PathAI, and Sysmex's newly closed BioMajesty chemistry acquisition all signal that portfolio boundaries across IVD are being actively redrawn

Siemens Healthineers: A Carve-Out Enters Active Preparation

Siemens Healthineers reported Q3 FY2026 (calendar Q2 2026) global Diagnostics revenue of $1.1B, a -5.5% comparable decline, including a -10.3% comparable decline in China specifically. Atellica placements continued, but were offset by legacy system retirements diluting reagent pull-through, while ongoing healthcare procurement price standardization in China continued to delay purchasing decisions.


Siemens Healthineers is advancing preparations for a formal carve-out of its Diagnostics division, creating organizational uncertainty during the transition. The carve-out process opens a real displacement window for competitors targeting expiring Atellica contracts while lab directors weigh Siemens' long-term diagnostics roadmap.

Abbott: Alinity's Installed Base Keeps Compounding

Abbott's worldwide Core Laboratory Diagnostics sales reached $1.4B in Q2 2026, up 3.2% on a comparable basis, with U.S. Core Lab accelerating to +7.5% comparable growth. Growth was propelled by robust hospital chemistry and immunoassay testing across the U.S. and Latin America, alongside continued high-throughput Alinity placement momentum.

Sysmex: Hematology Leadership Backed by a Newly Closed Acquisition

Sysmex reported global Diagnostics revenue of $814.2M in Q1 FY2027 (calendar Q2 2026), up 12.7% on a constant-currency basis. Regional performance was led by the Americas ($263.3M, +26.1% local currency) and EMEA ($257.1M, +11.0%), while China ($84.0M) declined 33.8% in local currency and Japan ($109.2M) grew 57.9% on a yen basis.


Americas outperformance was driven by next-generation XN-Series hematology installs and strong Brazil hemostasis growth. Japan sales were boosted by JEOL's BioMajesty clinical chemistry business, which closed as a Sysmex acquisition on April 1, 2026. The combination of rapid hematology and hemostasis installations with the newly integrated chemistry business establishes a highly competitive Core Lab portfolio, one that presents a direct market-share threat to Beckman Coulter and Abbott's hospital footprints.

Roche: China Drag Masking Ex-China Strength

Roche's Core Lab sales reached $2.2B in Q2 2026. On an H1 2026 basis, Core Lab CER growth was +4.0%, or +8.0% excluding the impact of China's healthcare pricing reform. Growth was led by immunodiagnostics and clinical chemistry assays, particularly infectious disease and bone health menus, with high-margin specialty menus cushioning Roche against China VBP headwinds.

QuidelOrtho: China Uncertainty Weighs on an Otherwise Stable Base

QuidelOrtho's Labs revenue reached $382.9M in Q2 2026 (+2.4%), with Immunohematology at $134.2M (+0.7%); total Diagnostics segment sales were $631M, up 2.0% on a constant-currency basis. Absent national VBP pricing guidelines, China customers are delaying purchases and drawing down inventory, though ex-China performance remains solid, with Labs up 9.0% and Immunohematology up 5.0%.


QuidelOrtho's high China VBP exposure has forced a defensive commercial posture across its mature immunodiagnostics and blood-banking accounts, and the company cut full-year revenue guidance to $2.52B–$2.60B on continued China uncertainty.

bioMérieux: SPOTFIRE's Growth Is Becoming Structural

bioMérieux's Molecular Biology revenue reached $415M in Q2 2026 (+2.5% organic growth), with the SPOTFIRE platform delivering $37M (+37.7% organic growth) and reagent sales up 40.0%. SPOTFIRE's installed base exceeded 7,200 units, with 400 new placements in Q2 alone, capturing share in decentralized urgent care settings and cushioning bioMérieux against a 6.0% decline in BIOFIRE respiratory panels, even as non-respiratory panels grew 7.0%.


bioMérieux's first-to-market syndromic point-of-care expansion lets urgent care clinics run a 20-minute multi-pathogen test, bypassing traditional central lab molecular delays. Both the FDA submission of the SPOTFIRE vaginitis panel and the CE marking of the first SPINCHIP assay landed in Q2

Abbott: Cologuard's Growth Compounds Into a Clinical Endorsement Advantage

Abbott's Cancer Diagnostics revenue reached $0.9B in Q2 2026, up 13.3% on a comparable basis, while Rapid & Molecular Diagnostics revenue was $0.8B, an organic decline of 8.0% on lower seasonal respiratory testing volumes. Cologuard's mid-teens user growth, on a growing base of both new and repeat users, drove the Cancer Diagnostics result, and in May the American Cancer Society reaffirmed Cologuard and Cologuard Plus as preferred colorectal cancer screening options.


Abbott's integration of Exact Sciences is building a dominant colorectal screening moat at the primary-care level, one that makes it difficult for pure-play molecular labs to compete on screening economics alone.

Cepheid's Q2 2026 non-respiratory revenue grew in the low double-digits organically, led by sexual health and hospital-acquired infection (HAI) assays on the GeneXpert system. A new Multiplex GI panel launch and several large hospital network account wins reinforced this menu-driven growth, offsetting expected post-pandemic respiratory testing declines.


Cepheid is converting COVID-only GeneXpert instruments into stable, high-margin clinical portfolios, and a next-generation high-plex syndromic platform now in development represents a direct competitive threat to bioMérieux's BIOFIRE franchise.

Roche's Molecular Lab sales reached $688.0M in Q2 2026, with H1 2026 CER growth of +3.0%, driven by 18.0% growth in transplant molecular testing. That non-respiratory specialty growth offset declines in respiratory testing and the cobas Liat near-patient system. Roche also reaffirmed its target of launching the Elecsys Interferon Gamma Release Assay for latent tuberculosis detection, further expanding its molecular screening menu.

Waters: BACTEC FXI Clearance Sets Up a Major Displacement Opportunity

Waters' Advanced Diagnostics Division (ADx) generated $521M in Q2 2026, up 7.0% organically, with acquired BDS delivering $449M (+5.0%) and the Clinical Business Unit (Mass Spec) delivering $72M (+14.0% constant currency). Early cost and revenue synergies from the BDS integration accelerated growth sequentially, and active pricing discipline drove a meaningful pricing lift.


The FDA-cleared BD BACTEC FXI blood culture system gives Waters an immediate advantage in sepsis diagnostics, with a 3-hour faster time-to-detection than incumbent platforms; it had already launched in Japan and Europe ahead of the U.S. clearance. With 4,500 U.S. instruments awaiting replacement, Waters has a large, well-defined displacement target directly threatening bioMérieux's blood culture base.

Roche Diagnostics: A $750M Bet on Digital Pathology

Roche's Pathology Lab sales reached $501.0M in Q2 2026, with H1 2026 CER growth of +11.0%, led by advanced staining and companion diagnostics. Advanced staining grew 9% and companion diagnostics grew 23% in Q1, as precision oncology therapeutic approvals continue to drive mandatory companion testing; VENTANA holds first-to-market companion diagnostics status through Roche's pharmaceutical co-development relationships.


Roche's Pathology Lab franchise is heavily insulated from molecular respiratory seasonality and central lab price wars. The $750M acquisition of PathAI, announced May 6 with up to $300M in additional milestones, creates a digital-clinical pathology barrier that pushes competitors toward basic anatomical staining rather than high-value companion diagnostics.

bioMérieux Microbiology: Localization Defends Share in China

bioMérieux's global Microbiology revenue reached $365M in Q2 2026, up 6.5% organically, on strong momentum in BACT/ALERT blood culture and VITEK REVEAL rapid antimicrobial susceptibility testing placements. bioMérieux has localized manufacturing of two BACT/ALERT systems and the VITEK Compact Pro in China, along with local blood culture bottle production, a strategy that maintains premium consumables positioning and is forcing global competitors to accelerate their own localization efforts.

Bruker's CALID group (mass spec and molecular diagnostics) posted Q2 2026 sales of $310.3M, up 8.6% versus Q2 2025's $285.8M, with 1H2026 CALID sales totaling $626.6M. The ELITechGroup molecular diagnostics integration is performing well ahead of business plan, placing high volumes of instruments and building a base for future consumables pull-through, while MALDI Biotyper platforms continue attracting strong interest for rapid bacterial identification.
